Staff within the Ongoing Complex Care department (OCC), Division of Aged Subacute and Complex Care, provide expert comprehensive health education to clients. The clients in the community are often at risk of readmission/presentation to hospital with multiple chronic conditions e.g. Diabetes, Hypertension, COPD, Cardiovascular disease, Congestive heart failure and Dementia.
The department is looking for enthusiastic Registered Nurses, interested in chronic disease management, to provide leave support on a casual basis for services under the Department of Ongoing and Complex Care. These services include Care coordination, Cardiac services, including Cardiac rehabilitation and Heart Failure rehabilitation, Pulmonary rehabilitation, both centre based and the home respiratory service, Neurological and Dementia services.
Training in the all or some of the disciplines will be provided to up skill and support appropriate casual staff to provide best practise, person centred care for clients and their carers. Successful applicants will have access to and will receive support from senior staff, CNCs and CNS2s, within all programs.
