Purpose of Position

Provide individualised excellent patient/client care within a community health/hospital/health service environment as part of a newly established Hospital in the Home (HITH) service to maximise the health outcomes for the patient/client.

Selection Criteria Current NSW drivers' licence

Demonstrated high level of oral and written communication skills.

Demonstrated capacity to identify and resolve conflict situations by managing own emotional responses and interacting with others in a personable and professional manner.

Demonstrated ability to proactively engage with patients/clients to understand their needs, enhance service delivery and act on situations when the norm is to wait for a situation to resolve itself.

Demonstrated ability to analyse problems and apply a range of options to develop solutions to avoid future crises in the short term.

Demonstrated application of professional and ethical boundaries when working with complex situations as part of a multi-disciplinary team

Registered Nurse with current Authority to Practice with APRHA including post- registration acute care experience.
