Purpose of Position

This position has coordination responsibility for the Transitional Aged Care Program within Narrandera and Leeton Health Services. The role encompasses service support and leadership of the program. The role includes day to day coordination and case management for the clients admitted to the program


This is a Permanent Full Time position, working 38 hours per week, and classified as Health Manager Level 1. This position is located in Leeton/Narrandera and belongs to Murrumbidgee Local Health District. Job share will be considered.

Selection Criteria Demonstrated ability to assess, plan and implement care for older people, including client focused goal setting.

Proven skills in multidisciplinary care coordination and interagency/interservice coordination

Demonstrated leadership skills in program implementation, including the ability to motivate and support others.

Demonstrated experience and successful practice in a health related discipline relevant to the position.

Demonstrated knowledge and understanding of the Transitional Aged Care Program (TAC)

Demonstrated high level of verbal and written communication skills, together with interpersonal and negotiation skills.

Computer literacy and use of Microsoft applications.

Evidence of Current Class C Drivers Licence and ability and willingness to travel for work purposes.
