Purpose of Position
  • The Registered Nurse will function within a multidisciplinary team to provide support and active nursing care for community nursing clients, including palliative care, HACC and TACP.
  • The community nursing service assists in the transition between hospital and home and facilitates people to manage at home more independently, whilst maintaining their optimal level of health.
  • The Registered Nurse will provide a high level of clinical nursing care which includes assessments, interventions, education and support in the community setting.

Selection Criteria 1. Registration with the Australian Nurses & Midwives Board

2. Demonstrated experience and knowledge of working with clients in a community setting including: HACC, TACP, palliative care, post-acute clinical management, wound care and immunisation.

3. Competency or willingness to acquire the clinical skills required for managing PICCS, PORTS, and NIKI syringe pumps.

4. Ability to plan, provide and coordinate holistic client assessment, monitoring and education; providing appropriate nursing interventions and care, encouraging self-management and making referrals for ongoing care needs.

5. A knowledge of the broad range of community based services and support organisations available in the local area to support clients to remain in their home environment.

6. Demonstrated experience working within a multidisciplinary team

7. Demonstrated effective interpersonal and communication skills, including computer based systems

8. Current unencumbered NSW Drivers Licence
