Purpose of Position

 

Triple I (Hub) provides an innovative service model, linking intake and assessment functions to connect clients to appropriate health services.  The role of the clinical coordinator at Triple I (Hub) provides high level critical analysis, clinical handover and exploratory skills to improve access to appropriate care for clients.  It is an integral role for the numerous Community Health services across South West Sydney Local Health District, improving efficiencies and quality in the transfer of care across primary and secondary services.


Triple I (Hub) provides and coordinates a single point of access (SPA) for admission and access to referral services.


These services include but are not limited to Community Health Nursing, Aged Care and Rehabilitation, Child and Family Health and Chronic Care.

Selection Criteria • Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA) with extensive experience in Child and Family / Community Nursing / Aged Care

• Highly developed interpersonal skills with a demonstrated ability to work as part of a multidisciplinary team in a dynamic and fast-paced environment as well as effectively communicate with health service providers and consumers using questioning, clinical reasoning and problem solving skills

• Demonstrated commitment and willingness to participate in ongoing professional development and quality improvement activities.

• Demonstrated organisational and time management skills, including prioritisation of tasks during peak demand periods

• Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.

• Demonstrated computer literacy with the ability to use Microsoft office applications

Demonstrated knowledge of community services and willingness to expand knowledge base

• Demonstrated skills in customer service and ability to deliver customer centred care in a high pressured working environment
