Purpose of Position

- Specialist clinical service provision for the Transitional Aged Care Program (TACP) for older people who have functional impairment and care needs upon discharge from hospital who are eligible for the SWSLHD TACP.

- Involvement in assessment, care planning and education to improve client functioning in the home environment.

- Ensure delivery of high quality, person centred care for clients of the service;

- Implement quality improvement across the TACP and Physiotherapy services

- Involvement in the overall development of the TACP program from a multidisciplinary

perspective

 

F/T Maternity Leave Relief up to the 9th December 2018 with a possible further 12 month extension

Selection Criteria Current National Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA)

Broad range of relevant physiotherapy clinical skills, including experience/skills relevant to community based therapy

Demonstrated ability to work independently and as part of a team

Demonstrated commitment to professional development, evidence based practice and quality activities

Effective verbal and written communication skills

Knowledge of community based aged care and rehabilitation services
