Purpose of Position

To provide pharmacy services after hours to the different wards within Nepean Hospital on a rotational basis. The pharmacist will provide medication review, counselling, drug information to all patients, carers and staff and detection and reporting of any adverse drug reaction, therapeutic drug monitoring and participation in the sterile suite and dispensary roster including participation in the on call roster.

 

Provide pharmacy afterhours and on call services and participate in the afternoon shift on a rostered basis

Selection Criteria Full AHPRA registration

Demonstrated satisfactory hospital pharmacy experience in an Australian hospital

Excellent communication skills both written and verbal with ability to communicate issues relating to medication safety

Demonstrated ability to work independently and cooperatively in a multidisciplinary team environment

Good clinical pharmacy practice including demonstrated therapeutic knowledge; experience in medication reconciliation and patient counselling
