Purpose of Position

This position plays a major clinical and complex care coordination role through the provision of expert clinical assessment, referral and coordination of health and community services for Aboriginal and Torres Strait (ATSI) people with severe chronic disease and complex care requirements living in the community who are at risk of deteriorating health that may result in declining quality of life and a hospital admission.

This position is required to work closely with patients/clients, carers their General Practitioners (GP) and Aboriginal Community Controlled Health Services, to identify needs (medical and non medical), strategies (self management, referrals to specialist services or community agencies etc.) and goals to improve their well being. It is the responsibility of this position to facilitate the implementation and evaluation of this shared care plan with the patients/clients and carers and their GPs. 

This position is based at RNSH Aboriginal Health Unit and will work across the Northern Sydney Health District to facilitate appropriate and timely care, support, monitoring and coaching for ATSI people with chronic conditions.

This position is Permanant Full Time and is classified as a Clinical Nurse Specialist Grade 2.

Selection Criteria Registered Nurse List A, AHPRA. Minimum of three (3) years clinical experience in Chronic Care with relevant post registration qualifications.

Extensive experience in complex care coordination requiring advanced clinical skills in patient assessment and care planning utilising both a multi-disciplinary and multi-sectorial approach in meeting the patient’s needs.

Demonstrated Aboriginal cultural awareness, sensitivity and knowledge of the issues affecting the health of Aboriginal people.

Highly effective communication skills both written and verbal. Including demonstrated competency in Microsoft Office products and relevant applications.

Demonstrated competency in clinical practice that promotes patient self-effacy through self-management, behaviour change and psychological support around a social approach to better Health for Aboriginal people living with chronic conditions and their carers and community.

Demonstrated knowledge and capacity to identify opportunities and implement strategies to enhance integration, communication and continuity of care for Aboriginal patients across acute/subacute care facilities, primary care providers and community based services.

Demonstrated ability to work effectively autonomously and in a team environment, utilising highly developed negotiation and time management skills.
