Purpose of Position

To facilitate through planning, the timely discharge/transfer of  patients ensuring they have access to the correct care, at the right time and in the right place, with minimum waiting time.   To work closely with the multidisciplinary team to coordinate care and ensure appropriate referrals are made and implementation of services or equipment occurs prior to the patient/client returning to the community.


This is a Temporary Part Time position, working 20 hours  per week until 30 June 2013, and classified as Registered Nurse. This position is located in Cootamundra and belongs to Murrumbidgee Local Health District.

Selection Criteria Registered nurse with five years nursing experience

Experience in, or demonstrated knowledge of integrated and coordinated primary and community health care, with experience working within a multidisciplinary team .

Demonstrated knowledge of internal and external service providers and their services available to assist and enable the transfer /discharge of a client to the community.

Knowledge and understanding of the Connecting Care program, 48hour Aboriginal followup and the Aged Care Assessment service for clients

Demonstrated experience in organising and conducting case conferences and multidisciplinary team reviews

Evidence of a Class C NSW Driver's Licence, and the abliity and willingness to travel for work purposes

Registered Nurse/Midwife with the Australian Health Practitioners Regulation Agency (AHPRA) together with demonstrated ability to meet the ANMC competencies for the Registered Nurse/Midwife.
