To facilitate through planning, the timely discharge/transfer of  patients ensuring they have access to the correct care, at the right time and in the right place, with minimum waiting time.   To work closely with the multidisciplinary team to coordinate care and ensure appropriate referrals are made and implementation of services or equipment occurs prior to the patient/client returning to the community.

This is a Temporary Part Time position, working 20 hours  per week until 30 June 2013, and classified as Registered Nurse. This position is located in Cootamundra and belongs to Murrumbidgee Local Health District.
