Purpose of Position

The Mental Health Shared Care Liaison Clinician aims to develop and foster positive working relationships with General Practitioners (GPs), with a focus on enhancing and increasing the participation of GPs in the improvement of the physical health of mental health consumers within the Sydney LHD. The position is based in a Community Health Centre and partners with the Central and Eastern Sydney PHN.

 The SLHD Mental Health Service provides assessment, care planning and intervention for consumers presenting with mental illness and/or mental health problems. The service works closely with carers and significant others involved with the mental health consumers, and with local GPs and other partners and service providers, to assist consumers to identify physical health goals, and promote pathways to improve physical health outcomes.

 The role includes those activities and competencies normally undertaken at an advanced practice level.

Hours of operation are Monday – Friday 8.30am – 5.00pm

Selection Criteria Recognised tertiary qualifications in Occupational Therapy, Social Work or Nursing with current registration with Australian Health Practitioner Regulation Agency (AHPRA) or eligibility for membership of Australian Association of Social Workers (AASW). For CNC, minimum of five (5) years full time equivalent post registration experience with at least three (3) years full time equivalent mental health experience

Demonstrated experience in developing strategic responses to the development of care pathways across diverse service delivery locations

Demonstrated understanding of the physical health needs of mental health consumers

Demonstrated ability to work effectively within a multi-disciplinary team and with external partners and agencies; effective interpersonal skills; excellent verbal and written communication skills, including experience in report and submission writing

Demonstrated skills in problem solving, and initiating and leading clinical practice development projects, including development and delivery of mental health education programs at an advanced level

Experience in conducting research and incorporating findings of research into the provision of clinical services

Demonstrated advanced knowledge and skills in the use of information technology systems

Current NSW Drivers licence (P2 licence acceptable)
