Purpose of Position

Sustained Home Visiting Program provides a flexible, family focused service which maximises the potential of all children from pregnancy to 2 years of age and their families, in particular where risk factors are identified and families have high needs.

Selection Criteria Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) and Child & Family Health Nursing recognised qualifications.

Demonstrated knowledge and understanding of early intervention, theoretical evidence supporting sustained health home visiting programs and the Family Partnership Model.

Demonstrated experience of working in partnership with vulnerable families.

Commitment to undertake and complete training requirements to work within the sustained health home visiting model.

Demonstrated high level of communication, liaison, conflict resolution skills, networking skills, and the ability to work successfully with a range of stakeholders.

Demonstrated ability to work effectively as a member of a multidisciplinary team and in an autonomous role

Demonstrated high level competency with computer literacy and experience with electronic medical records.

Unrestricted NSW Drivers Licence (P2 Licence acceptable).
