Purpose of Position

To deliver a quality Sustained Nurse Home visiting program using the evidenced based MECSH model. This provides a flexible family focused service which maximises the potential of all children from pregnancy to 2 years. Home visiting  In particular where risk factors are identified and families have high needs.

Internal to Sydney Local Health District

Selection Criteria Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) and Child & Family Health Nursing recognised qualifications.

Demonstrated experience of working in partnership with vulnerable families

Commitment to undertake and complete training requirements to work within the sustained health home visiting model

Demonstrated high level of communication, liaison, networking, and the ability to work successfully with a range of stakeholders.

Demonstrated ability to work effectively as a member of a multidisciplinary team and in an autonomous role

Demonstrated high level competency with computer literacy and experience with electronic medical records.

Unrestricted NSW Drivers Licence (P2 Licence acceptable).
