Purpose of Position

Purpose of Position

The Carer Project Co-ordinator will share responsibility for Carers’ Program initiatives in the Sydney Local Health District (SLHD) with other team members, by:

  • Assisting in the promotion of an environment in the SLHD that better supports and includes carers;
  • Supporting the improvement of access and service delivery to carers by mainstream staff;
  • Raising awareness amongst SLHD staff of the role and needs of carers in the contribution they make in the provision of care to SLHD health care consumers
  • Developing and implementing components of the District Carer Action Plan that are the responsibility of the SLHD Carers’ Program Concord H site, in association with other team members and under the supervision of the Service Manager.

Selection Criteria Degree in Social Work or equivalent providing eligibility for membership of the AASW and current experience at Level 2 or higher

Demonstrated understanding of issues related to the support of carers and knowledge of resources for carers

Extensive experience and/or knowledge and skills in planning, implementation and evaluation of projects, including an understanding of health promotion principles and frameworks

Experience in providing training and education

Demonstrated communication, decision making and negotiation skills

Ability and experience in consumer(Carer)advocacy

Skills, knowledge and experience in using software programs such as microsoft word, excel, publisher, powerpoint and outlook (or similar email software)

Current NSW Driver's Licence (P2 licence acceptable)
