Purpose of Position

 

The primary role of the comprehensive assessor is to independently and comprehensively assess the care and support needs of frail older people and to facilitate access to available Commonwealth subsidised aged care services that are appropriate to their needs.

The comprehensive assessment encompasses the restorative, physical, medical, psychological, cultural and social dimensions of the clients care needs with the provision of a choice of appropriate services to meet their needs.

The SNSW LHD amalgamated ACAP operates out of five service centres, from time to time the incumbent may be required to work at other sites.

The ACAP and Regional Assessment Service (RAS) are together considered end-to-end My Aged Care (MAC) Assessment services.  The ACAP comprehensive assessor will work closely with both NSW LHD RAS Assessors and as required liaise with partner RAS organisations.

 

  

This is a Permanent Part Time position, working 28 hours per week, and classification is dependent on qualifications (either Registered Nurse, Psychologist, or Allied Health Professional 1/2). This position is located in Goulburn which belongs to Southern NSW Local Health District.

Selection Criteria
1. AHPRA registration and relevant qualifications as a Registered Nurse, Allied Health Professional or Psychologist.

2.Demonstrated capacity to stay informed of current trends in Aged Care service delivery.

3.Demonstrated ability to work both independently and collaboratively within a team, including an ability to work across discipline boundaries and in a multidisciplinary team.

4.Detailed knowledge of Australian Government ACAP Guidelines 2015, and legislative requirements under the Aged Care Act 1997 also My Aged Care Assessment & Portal User guides.

5.Demonstrated high level of verbal and written communication skills, including developed negotiation and conflict resolution skills.

6.Demonstrated ability to organise personal work priorities and manage time effectively.

7.Knowledge of computer systems and the use of databases relevant to the ACAP.

8.Current unrestricted Driver’s License and the ability and willingness to travel for work purposes.
