Purpose of Position
  • To provide advanced level clinical nursing care / interventions as authorised by the NSW Nurses and Midwives Registration Board
  • To provide clinical care for a diverse range of geriatric conditions that affect wellness within a defined and agreed scope of practice and clinical guidelines
  • Provide advanced clinical skills and assessment / intervention and consultation
  • Work collaboratively with the staff to identify frail aged and chronically ill patients requiring complex nursing care planning and advanced clinical nursing hospitalisation or readmission
  • Ensure the coordination of the care of frail aged clients across multiple providers, establishing referral pathways and protocols in consultation with key internal and external Community Service partners
  • Liaise with all staff to support and promote mainenance of self-management programs for aged care patients
  • Work with the Nepean Blue Mountains Primary Health Network to develop 'shared care' partnerships with GPs
  • Contribute to GP care plans and work collaboratively with GPs to co-manage clients and access Chronic Disease Management items were appropriate
  • Establish communication links and work collaboratively with residential aged care facilities to support promote management of aged care patients
  • Contribute to broader goals of hospital avoidance and improvement of the patient journey through early intervention and patient-centered care planning
  • Contribute to the broader body of aged care knowledge across the Nepean Blue Mountains Local Health District

Selection Criteria Authorised by the Nurses and Midwives Board of NSW, pursuant to Section 19A of the Nurses Act 1991, to practice as a Nurse Practitioner or currently working towards the same with the ability to complete all requirements in 2 years

Minimum of five years full time equivalent post registration experience, with a minimum of three years full time (equivalent) within the acute or community aged care environment

Demonstrated 5000 hours of advanced practice in the acute aged care environment and documentation that supports attainment

Demonstrated excellent leadership, teaching, interpersonal abilities within the specialty and the profession

Demonstrated clinical excellence in aged care and proficiency in geriatric assessment skills

Demonstrated excellent written and oral communication skills, computer skills and demonstrated critical thinking skills

Demonstrated ability to work autonomously and as part of a multidisciplinary team

Current NSW State Driver's Licence including P1 or above
